Our client onboarding is for YOU and US to determine:

Initial Inquiry & Discovery Call

Introduce The Savvy Senior, our values, and the non-medical services we provide.

Learn about the client, their interests, daily routines, and current support needs.

Discuss the reasons you are seeking companionship or senior support services.

Review the types of activities and assistance that may be appropriate based on the client's preferences and goals.

Explain the scope of our services, including what we can and cannot provide.

Discuss preferred days, times, and frequency of visits.
-
Identify any mobility, communication, safety, or accessibility considerations that will help us provide appropriate support.

Answer questions about our caregivers, scheduling process, service fees, and company policies.

Explain our onboarding process, including the in-home visit, service agreement, and development of a Personalized Aging Well Service Plan.

Discuss the next steps should you choose to move forward with services.

Savvy Home & Lifestyle Visit

A clear understanding of the client's goals, interests, routines, and preferences.

Completion of an initial assessment to identify the client's non-medical support needs.
-
Discussion of the client's lifestyle, hobbies, social interests, and meaningful activities.
-
Identification of any mobility, communication, or accessibility considerations that may affect service delivery.
-
A general review of the home environment to identify any safety considerations relevant to providing services.
-
Confirmation that the requested services fall within the scope of our non-medical support services.

Discussion of client expectations, family involvement and preferred methods of communication.

An opportunity for the client and family to ask questions and ensure they have a clear understanding of our services, policies, and procedures.

Determination of whether our organization is the right fit to meet the client's needs and expectations.

Identification of the qualities, personality, interests, and experience that will help us make a compatible companion match.

Review of service fees, scheduling, and the Service Agreement.

Personalized Aging Well Service Plan

Client and emergency contact information.

A description of the agreed-upon non medical services to be provided.

Review the client's Personalized Aging Well Services Plan and identified goals.

Days, times, frequency, and duration of scheduled visits.
-
Service fees, payment terms, and invoicing procedures.
-
Cancellation, rescheduling, and missed visit policies.
-
Procedures for after-hours communication and non- emergencies.
-
Client and family responsibilities, including providing a safe environment for service delivery.
-
The organization's responsibilities for providing professional, respectful, and reliable services.
-
Privacy and confidentiality commitments.
-
Consent for communication with designated family members or representatives, where authorized by the client.
-
Procedures for reporting concerns, complaints, or compliments.

Circumstances under which services may be modified, suspended, or discontinued.

Acknowledgement that The Savvy Senior provides non medical companionship and support services and does not provide medical, nursing, or personal care requiring regulated healthcare professionals.

Service Agreement

The Service Agreement will be reviewed with the client and/or their authorized designate before services begin to ensure all questions are answered and expectations are clearly understood.

The agreement may be updated whenever there are changes to the client's service needs, schedule, fees, or other terms of service. Any significant amendments will be discussed with the client or authorized representative and documented in writing.

A signed copy of the Service Agreement will be maintained in the client's file, and a copy will be provided to the client for their records.

Service Review & Adjustments
